Facilitator Guide

    Select the top 2–3 prioritized problems from Phase 2 for root cause analysis.

    Use the "5 Whys" technique: ask "Why does this problem occur?" repeatedly until the root cause is identified.

  3. 3

    Alternatively, use a fishbone (Ishikawa) diagram to categorize causes (people, process, policy, environment).

  4. 4

    Encourage participants to distinguish between symptoms and root causes.

  5. 5

    Map the relationships between causes to identify systemic issues.

  6. 6

    Validate the root cause map with the group before finalizing.

  7. 7

    Identify which root causes are within the scope of regulatory policy to address.
